Emergency Support Plan

What would happen if you were suddenly unable to care for someone you support?

The LLR Carers Emergency Support Plan helps you prepare for this. It gives you a place to record important information about the person you care for, so that someone else can step in and provide the right support if needed.

You can complete the plan yourself and only need to fill in the sections that are relevant to you. You can complete it online or download and print a paper copy.

Once it’s complete, keep it somewhere safe and easy to find, and let the people you trust know where it is.
